Overview:
Intercare Recruitment is seeking a dedicated Full-time Physical Therapist to join a short-term and long-term care rehabilitationcenterwith 90 beds, the centeris a medium-sized facility committed to providing exceptional rehabilitation services to its residents.
Responsibilities:
Provide physical therapy services to residents in various units, including short-term rehabilitation and long-term care.
Develop and implement individualized treatment plans based on resident needs and goals.
Collaborate with the interdisciplinary team, including physicians, nurses, and other therapists, to ensure comprehensive care.
Monitor and document resident progress, adjusting treatment plans as necessary.
Educate residents and their families on treatment plans, home exercises, and injury prevention.
Participate in team meetings, case conferences, and other collaborative efforts to improve resident care and outcomes.
Maintain a safe and therapeutic environment for residents.
Qualifications:
Licensed Physical Therapist
Strong interpersonal and communication skills
Ability to work effectively within a team environment
Commitment to providing high-quality care and services to residents
