2nd Chance Clinic is an outpatient addiction treatment center in Lexington serving adults in the greater central Kentucky area. We are currently seeking an experienced, self-motivated Targeted Case Manager for Substance Use and Co-Occurring Disorders to become part of our Behavioral Health care team which includes providers, counseling, and peer support staff.
In this position you will assume primary responsibility for all case management functions for clients served including assessment, service planning, coordination, advocacy, and monitoring through a comprehensive service plan.
Duties
Complete new patient assessments. Complete new patient biopsychosocial assessments and preliminary screeners, and review client’s strengths and needs.
Determine level of care need from assessment and discuss with patient to ensure patient’s understanding. Evaluate with patient that care needs meet their basic health, safety and overall welfare needs.
Ensure that patients understand their rights regarding confidentiality.
Plan services based on assessments. Facilitate access, quality, and delivery of necessary services and provide follow-up to patient.
Provide psychoeducation and resource information while acting as a positive role model, providing patients with guidance and support.
Promote and foster effective relationships, both inside and with outside agencies for continuity of care.
Ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.
Maintain client confidentiality in accordance with federal, state, and agency standards. Practice ethical standards.
Respond to all patient grievances in accordance with written policies.
Participate in quality assurance and utilization review.
Attend quarterly all staff meetings and monthly departmental meetings.
May perform other duties as required.
Requirements
Bachelor’s degree in human services field plus 1 years’ experience in behavioral health case management (or an equivalent of education and experience).
Complete Targeted Case Management Core training and SUD training provided by an approved provider of the Department of Behavioral Health, Developmental and Intellectual Disabilities within 6 months of employment.
Maintain required continuing education units per state regulations for targeted case managers.
Excellent oral and written communication skills, basic computer skills in word processing, email and Internet use, strong interpersonal skills, and professionalism.
Current certification in BLS, non-violent crisis intervention, and suicide intervention a plus.
