Social Worker

Job Description
Assists the Director of Social Services in developing, implementing, and maintaining Quality Assurance programs for Social Services.
Develops and maintains professional relationships with BV staff, physicians, residents, families, community resources, and third-party payers.
Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process
Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly.
Assess residents variouis health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.
