Job Description
I. General Summary
Under general supervision provides leadership and expertise in social work interventions for patients and their families to enhance comprehensive, integrated and uninterrupted care in the hospital and to have continuity of care in the community. Demonstrates positive interpersonal relations with patients, families, visitors, and co-workers in accordance with UMMC and departmental policies and procedures. Through advanced practice skills mobilizes resources to reduce risk, and serves as ambassador between hospital and community.
II. Principal Responsibilities and Tasks

Serves as a content-area expert for the manager and staff and serves as a resource regarding best practices.
Identifies gaps in knowledge, skills or abilities and develops and provides educational resources for the department and other staff as needed.
Provides consultation and clinical supervision staff in their practice area on difficult cases.
Develops relationships with community resources and interprets services available at those programs to clients, families and unit/department staff.
Develops and leads performance improvement activities.
