This position is a full time (0.8 - 1.0 status), day shift RN Nurse Navigator position in the CPG Surgical Specialists Department.


POSITION SUMMARY


The focus of the RN Nurse Navigator position is to plan, coordinate, and provide specialized wound, ostomy, and continence care along with wound care team including education for patients and families. Standardize care for patients with critical limb ischemia. Perform risk assessments, facilitates appropriate testing ordered by license providers, and supports, coordinates, and educates patients and their families as it relates to disease, procedures and testing. Serve as resource person on nursing practice in skin, wound, ostomy, and continence care for hospital staff and community. Coordinate, implement, and maintain a comprehensive pressure sore prevention program in hospital. Coordinate comprehensive ongoing education and staff development in all aspects of skin, wound, ostomy and continence care. Help coordinate with measuring outcomes and quality improvement. Plan, coordinate, and provider Anorectal Manometry testing as well as collaborate with out-patient wound care team.


PRIMARY (ESSENTIAL) DUTIES



  • Collaborates with the physician to optimize patient care-assists with performing patient assessments and treatments.
  • Educates patients and their families about their condition.
  • Coordinates patient care needs with other interdisciplinary healthcare team members.
  • Communicates with referring providers regarding patient appropriateness.
  • Recommends and implements program guidelines.


EDUCATION AND EXPERIENCE


Minimum Education:



  • RN License Required, BSN preferred.
  • Bachelor of Science in Nursing
  • One (1) year acute care experience.
  • Proficient in English.
  • Excellent verbal and written communication skills, and computer skills required.
  • Must obtain BLS for Healthcare Provider within 90 days of hire.


Preferred Qualification(s):



  • Certification in specialty preferred.
  • BLS
  • Wound care and ostomy experience preferred.


Minimum Work Experience:



  • One (1) year acute care experience.


Required Skills:



  • Includes the ability to educate physicians, staff, and patients in the arena of health and related diagnostic studies.
  • Must have current computer skills and ability to navigate several systems for IT systems.
  • Must have excellent communication skills, both written and verbal.
