Job Description
Utilizes nursing assessment skills to identify clinical, emotional, environmental variable that may impact patients’ needs and adjustment to diagnosis and provides necessary interventions and referrals.
Develops and maintains a written plan of intervention for patients with positive lung nodule findings.
Implements plans/programs to meet the needs of the patients.
Coordinates and facilitates treatment objectives for the patient.
Develops informational and educational materials for this patient population.
Serves as liaison to services and resources within the hospital and the community.
Serve as a facilitator of selected support groups.
Participates in speaking engagements that support the service line and patient navigation.
Demonstrates sensitivity to the needs of customers, staff, physicians, patients and families.
Responds to concerns, questions, and other feedback in timely and thoughtful manner.
Identifies patients in need of ongoing diagnostic interventions and related services;
and facilitates initialencounter in multidisciplinary clinic.
Works collaboratively with all physicians/specialists/ oncology navigators involved in the patient’s care.
Communicates the plan to primary care physicians and obtains consensus for participation in this service.
Advocates for the use of Holy Cross Health specialty services in the next phase treatment plan and provides linkage of those services.
Provides emotional support and education to patients during each phase of treatment.
Acts as a conduit to resources as needed by patient.
Organizes and actively participates in a regular multidisciplinary care conference.
Maintains and builds follow up date systems for patients referred to the HCH.
Co-leads a multidisciplinary group that develops education tools and strategies for lung health learning needs across the continuum.
Serves as resource advocate to assure access to appropriate lung health resources
Demonstrates knowledge of biopsy procedures performed and has ability to explain them.
Demonstrates proficiency in counseling patients who have been recommended for biopsy.
Obtains physician referrals so that patients may have their biopsy procedure expedited.
Follows up on each patient’s pathology results.
Provides appropriate teaching, education to patients.
Provides education and mentoring to clinic colleagues related to lung health, customer service and continuity of care.
