Registered Nurse, Rn (Home Health Visits)

Responsibilities:
- Manage the medical needs of home health patients using all available resources
- Work with other disciplines to maximize patient outcomes and be a patient advocate
- Communicate with the primary doctor, Clinical Team Manager, and other team members to manage patient care
- Provide professional and compassionate care to patients during home health visits
Qualifications:
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
- 1 year nursing experience preferred
- Acceptance of philosophy and goals of the agency
- Ability to exercise initiative and independent judgment
