This position is responsible for but not limited to:
Complete documentation and accurate records as required including:
Program guides, participation tracking, monthly statistics, Intake documentation, Quarterly documentation, MDS/RAPS, regular care plan updates based on residency status, progress notes, all assessments needed to determine residents recreation and rehabilitation needs.
Plan and implement programs based on resident assessments, offering new, creative and innovative recreation programs to residents on an ongoing basis
Plan and implement special events, community based programs and outings
Evaluate and make adaptations to programs as appropriate
Complete equipment assessments and ensure that resident has access to appropriate assistive devices as required(i.E. wheelchair, walker)
Participate in department meetings, unit meetings, and multidisciplinary care conferences.
Ensure effective communication and good human relations with residents, residents’ families, volunteers and staff
Ensures that posters, notices, and calendars are up to date, attractively displayed and easy for residents to read.
Initiate media notices re:
creative programming in conjunction with the Programs Manager
Develop and maintain community contacts and resources.
Work with different community based agencies and groups to deliver programming to residents
Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
Required to participate in the improvement of the indicators and achieve satisfactory results.
Other duties as assigned and or indicated in Job Task Inventory
Qualifications

Post-secondary diploma or degree in recreation and leisure studies, therapeutic recreation, kinesiology or other related fields from a community college or university or enrolled in the same required. If enrolled, must complete program within 3 years of hire date,
Demonstrated written and verbal communication skills, as well as excellent documentation skills,
Knowledge and demonstrated experience in one-one/group program planning, implementation and evaluation for both well and the cognitively impaired,
Ability to work independently and ability to manage time effectively,
Ability to plan, implement, evaluate new ideas, resident programs and volunteer initiatives,
Knowledge and/or experience in working with the geriatric population especially those with cognitive impairments,
Current CPR/First-Aid and Food Handler’s certification,
Knowledge of MDS/RAI would be an asset,
GPA and Montessori certificates would be an asset.
Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references.
