Primary Care Physician

Responsibilities:
Perform comprehensive in-person assessments and collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans for participants.
Oversee all aspects of participant care in nursing homes and hospitals, including admissions, discharges, and coordination with medical specialists.
Provide direction and support to Nurse Practitioners and other IDT members to ensure effective care coordination and delivery.
Deliver primary care services, including assessment, planning, and continuous medical coverage to participants.
Collaborate with community-based physicians and IDT in discharge planning from acute or long-term care settings.
Actively participate in IDT meetings, communicate participant changes, and coordinate 24-hour care delivery.
Maintain regular visits to nursing home participants based on their needs.
Participate in rotating night and weekend call schedules as required.
Requirements
Qualifications:
Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
Master’s Degree or Doctorate Degree in Medicine.
Board Certification in Internal Medicine or Family Practice.
Board Certification in Geriatrics preferred.
Active California Physician License without Restrictions.
State CDS Certification and DEA Registration.
