Our organization is seeking a mission-driven and compassionate Primary Care Nurse Practitioner to join our interdisciplinary team (IDT) and provide direct medical care to PACE (Program of All-Inclusive Care for the Elderly) participants. As a Primary Care Nurse Practitioner, you will work collaboratively with the IDT to create and implement care plans aimed at improving the health outcomes of our participants.
Key Responsibilities:
- Provide direct medical care to PACE participants
- Complete sections of assessments and care planning
- Manage a panel of participants
- Initiate, evaluate, review, and change orders for medications and diagnostic studies or treatments with approval
- Assess, initiate, and coordinate care on a daily basis under supervision
- Document all participant interactions, assessments, and next steps in the EMR within the specified timeframe set by the center
- Make referrals to specialists with approval and request consults as needed
- Perform history and physicals and participant teaching
- Order, interpret, and follow up on lab tests and results
- Perform selected procedures
- Recognize and escalate abnormalities as needed
- Liaise with other team members for participant’s care, including behavioral health and pharmacy
- Evaluate and treat participants where they are located, whether that be in the center, home, virtually, SNF etc
- Facilitate advanced planning discussions with participants
- Document chronic conditions via HCC capture
Schedule and Shift Details:
- 40 hours per week, no on-call. Schedule will vary.
