Responsibilities:
Provide comprehensive physical therapy services to residents.
Develop and implement individualized therapy plans.
Collaborate with other healthcare professionals to ensure quality care.
Participate in interdisciplinary team meetings to discuss and coordinate resident care.
Document resident progress and modify treatment plans as necessary.
Educate residents and their families on continued care and exercises to be done at home.
Qualifications:
Licensed Physical Therapist.
Strong interpersonal and communication skills.
