Responsibilities
Assist the Physical Therapist in developing and implementing an individualized care plan.
Implement the care plan by delivering prescribed exercises and education under the direction of the Physical Therapist.
Observe, record and report changes in the client's condition and progress to the Physical Therapist and care team.
Provide education to the client and their family to ensure safety and proper use of equipment and exercises.
Adhere to all safety regulations and company policies and procedures.
Document daily progress notes and other required documents accurately and in a timely manner.
Participate in interdisciplinary team meetings as needed to develop, review, and update care plans.
Requirements
Physical Therapy Assistant degree from an accredited institution.
Active State of Michigan PTA license in good standing.
Minimum of 1 year experience as a Physical Therapist Assistant with home health care experience preferred, but not required.
Proficient knowledge of therapy administration techniques and equipment.
Excellent communication and interpersonal skills.
Ability to work independently, while also serving as an integral part of a team, to manage multiple cases.
Valid driver's license and reliable transportation.
