At Seasoned Recruitment, we are seeking a compassionate and dedicated PTA to provide quality home health care to patients in their own homes. As a Home Health Physical Therapist Assistant, you will work closely with a team of healthcare professionals to deliver personalized treatment plans and help patients regain their mobility and independence.
Responsibilities:
Assist in providing physical therapy services to patients in their homes
Collaborate with the physical therapist to develop and implement individualized treatment plans
Guide and support patients through therapeutic exercises and activities
Monitor and record patient progress during therapy sessions
Educate patients and their families on proper techniques for exercises and activities
Assist with modalities and other treatment interventions as directed by the physical therapist
Maintain accurate and timely documentation of patient treatment and progress
Communicate effectively with the healthcare team to ensure coordinated and comprehensive care
Requirements
Qualifications:
Graduation from an accredited Physical Therapist Assistant program
Licensure or certification as a Physical Therapist Assistant in the relevant state
Prior experience in home health or a related setting is preferred
Strong interpersonal and communication skills
Ability to work independently and as part of a team
Compassionate and patient-centered approach to care
Valid driver's license and reliable transportation
