Job Description
Under supervision, the Outreach Specialist will work with clients to ensure that they keep medical and non-medical appointments necessary to qualify for Supplemental Security Insurance (SSI) Benefits by meeting clients in the community and accompanying them to the appointments at CAAP and Public Health and other community providers. This may include contacting clients in hotels in which they live, or if homeless, in shelters or sites where they are known to spend time. The Outreach Specialist will assist the client in completing forms and will complete Outreach Questionnaires to document clients' functional limitations.
ESSENTIAL DUTIES AND FUNCTIONS
According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list;

Advise clients regarding health care and other facilities available to them assist clients in utilizing such services;
make follow-up contactswhen required.
Serve as liaison between the professional staff and the community, providing information concern the living conditions of the clients, presence of lack of evidence of psychoactive substance use and paraphernalia, and the level of assistance the client requires to keep appointments.
Conduct effective outreach to locate clients not completing case management activities.
Effectively engage and accompany clients to necessary appointments and ensure that the client successfully completes the activities during the scheduled appointment.
Develop effective working relationships with community-based organizations, case managers, program workers etc.
Assist the client in completing forms when necessary.
Document outreach efforts and observations of client functional limitations in progress notes and on Community Outreach questionnaires.
Adhere to all client confidentiality and Quality Assurance standards.
Assists in gathering and evaluating data concerning the program to which assigned;
may perform incidental clerical duties such as keeping records, answering the telephone and arranging client appointments.
May transport ambulatory patients between their homes and clinics, hospitals or other social agencies;
may transport staff members to meetings with administration approval;
reports malfunctions ofthe vehicle to supervisor.
Perform administrative and other duties as required.
