Job Description
This position is a casual role at Lake Forest Hospital. Minimum requirements of 2 weekend days per month plus 4 hours weekday per month. Vacation requirements of1 legal vacation and 1 “soft vacation” in the summer AND 1 legal vacation and 1 “soft vacation” in the winter.

This is a flat rate position.

Responsibilities:
Develops and implements occupational therapy programs aimed at maximizing function while educating patients regarding their limitations.
Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
Establishes appropriate goals based upon functional limitations, diagnosis, and patient's home setting.
Communicates treatment program to patient, family and other interdisciplinary personnel;
provides emotional support as needed by patient and family.
Coordinates and implements the treatment plan.
Treats patients utilizing therapeutic exercise, functional activities, manual techniques, adaptive equipment, and energy conservation.
Evaluates the home setting and makes recommendations and appropriate referrals to assure safe and efficient function within the home.
Re-evaluates patients to maintain effective plans of care and need to recertify for additional cert periods.
Plans for patient discharge by conferring with physicians, patient, interdisciplinary team and family;
reviews and finalizes appropriate exercise program and practices for independent and safe care.
Documents initial evaluations, progress notes, discharge notes and discharge summaries in patient record accurately and within time frame specified by department policy.
Develops and presents educational programs for the department and the community as appropriate and as needed.
Instructs, supervises and evaluates the Home Health Aide at least every 14 days as appropriate.
Is responsible for maintaining CEU licensure requirements.
Reports safety hazards and makes suggestions to improve safety.
Work practices consistently reflect understanding of and adaptation to recognized safety standards such as National Patient Safety Goals and OSHA standards.
