JOB SUMMARY
Responsible for program coordination, staff scheduling, staff mentoring, and provision of ADL care in Traditions Memory Care Neighborhood, functioning as a senior member of the Memory Care Team.
ESSENTIAL FUNCTIONS
Precepting new employees by modeling tasks, roles and responsibilities, policies, and procedures.
Acts as a resource for current employees for any assistance they may need (i.E., computer or data entry assistance, problem solving for work-related issues, scheduling.
Resident appointment escort scheduling (Coordinating w/ Appointment Coordinator)
Organizing and keeping track of resident appointment calendar for on/off unit medical &
salon appointments
Create and update monthly staff schedule for approval by the Memory Care Manager
Creating and updating monthly resident assignments/ medication pass schedule
Re-staffing for staff call-offs / PTO’s / emergencies
Entering schedule into Kronos and making updates as needed
Creating, updating, and providing copies of all logs and reports used in unit
Assists Memory Care Manager with maintaining licensing compliance with documentation, binders, and forms
Incontinent / medical supply orders and inventory (Coordinating w/ Central Supply)
Office supply ordering for unit
Preparing / updating resident emergency packets including ongoing medication list updates.
Preparation for new admissions (gathering information to file in appropriate binders for providing care, bio/history, releases, and inventory sheet of resident belongings. Ensures door and assigned cabinets/shelves are labeled with resident’s name)
Coordinates moves in/out of the unit (makes work orders, coordinates where furniture will go, makes sure clothing is labeled appropriately &
inventory of belongingsis completed.)
Filing documents (i.E., Service Plans, Resident Info, Resident Inventories)
Collecting &
entering monthly weights/vitals into MatrixCare
Collaborates with Memory Care Manager on updating resident Service Plans bi-annually, COC’s &
inputting changes to the service plan as needed.
Assists with creating new service plans as needed for new admissions.
Assists with escorting residents to appointments, leading activities, answering phone calls, and covering the floor as needed.
Implements Memory Care Program and engagement programs as directed by the Memory Care Manager
Monitors staff issues on the floor and informs manager of any staff issues that may need to be addressed by the memory care manager to the team members immediately to ensure that the community is running smoothly.
Coordinates with Recreation Therapist on ideas for activities/ outings for monthly calendar and daily calendars for residents.
Works with staff to ensure timely food orders and kitchen inventory
Maintaining staff and resident Covid testing schedules and vaccination records with oversight of Memory Care Manager.
Requirements
SKILLS, ABILITIES AND EXPERIENCE
Experience in geriatrics and caring for residents with dementia.
Excellent customer service and organizational skills
Sufficient verbal and written communication skills to ensure clear communication with residents, staff and for documentation as required.
Knowledge of and experience with Excel and scheduling
Maintains confidentiality of appropriate resident care information to assure their rights are protected.
Reports all incidents/accidents, unsafe and hazardous conditions/equipment immediately to the memory care manager. Follows established safety rules and regulations. Maintains work area in a clean, orderly, and safe manner.
EDUCATIONAL REQUIREMENTS/LICENSES/CERTIFICATES
Any combination of education and experience that would provide the necessary knowledge and abilities listed.
