Licensed Clinical Social Worker

The Licensed Clinical Social Worker intervenes to provide clinical social work and complex discharge planning to patients and their families who have psychosocial needs. They assist with eligibility determination for social programs and funding sources, and help them qualify for community assistance from various special funds and agencies. Additionally, they provide crisis intervention and mental health assessments to patients and families with psychosocial needs. The worker also coordinates and facilitates the development of a multidisciplinary discharge plan for the care of high-risk patient populations. This role involves participation in an interdisciplinary team, including physicians, case managers, staff nurses, and other care team members, providing services to individuals from at-risk populations and ensuring that psychosocial issues are addressed and treated across the continuum of care.
