About the Role:
The Case Manager manages an average caseload of 18 to 20 families at risk of homelessness or recently housed. The
Case Manager will support the housing first model with a trauma-informed care approach that will maximize the
Likelihood of client success and long-term self-sufficiency. The position requires a self-starter as the responsibilities
Include:
providing ongoing, community-based services, identifying, prioritizing, and addressing immediate needs,
Developing money management skills, supporting gainful employment search, coordinating training, and
Educational goals, and address mental health and substance abuse issues. The Case Manager will serve as the
Primary point of contact for all assigned client services and provide guidance and support to clients working toward
Self-sufficiency.
How you will contribute:
- Perform assessment of each family's housing history and barriers, physical, mental health and social needs,
Educational and employment needs, financial management skills, strengths and needs, and other necessary
Areas.
- Meet with assigned families regularly and provide appropriate services and referrals geared towards housing
Stability.
- Assure all relevant information pertaining to clients is disseminated to all appropriate partners in a timely
Manner.
- Support clients in the creation and maintenance of financial budgets and support clients in implementing
Money management techniques.
- Participate in meetings with representatives from other agencies including Faith-Based and Community
Based Organizations to deliver wrap-around services to clients.
- Ensure that clients achieve the objectives of maintaining long-term housing, by complying with the terms of
Their lease agreement and have an Individualized Housing Sustainability Plan that helps them achieve their
Desired level of self-sufficiency.
- Mediate household, roommate, and/or landlord-tenant disputes that threaten housing stability.
- Serve as point-of-contact for landlords and maintain positive landlord relationship.
- Develop and maintain a Resource Directory that includes state and local service providers.
- Maintain thorough case documentation in HMIS and client files and comply with agency and standard best
Practices.
- Proficiently utilize Google Suites applications (email, calendar, drive, docs, etc) as a part of day to day work
Requirements
- Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees deployed
Staff and involved agencies.

- Carry out other duties as assigned by Assistant Director and/or Program Director.
- Prepare monthly reports to Supportive Services Manage
Requirements
Required:
- Bachelor's Degree in Social Work, Psychology, Human Development, or related degree or commensurate
Experience.
- Three years of experience in service delivery with low and moderate-income families.
- Ability to prioritize competing priorities and make sound judgment.
- Strong written communication skills.
- Ability to advocate, assist in problem-solving, and achieve results for families
- Excellent time management and organizational skills
- Valid Driver’s License
- Willingness to work evenings and weekends as needed.
Preferred:
- An MSW or master's degree in human services/ related field
- Experience with providing services to clients experiencing homelessness, Mental Health or substance abuse issues
- Ability to speak a language other than English at a proficient level (i.E. Spanish, Farsi)
- Comfortably with and quickly adapts to change.
- Strong public speaking and communication skills.
Physical Requirements:
● Annual TB Test is required
● Ability to sit or stand for long periods
● Ability to lift items weighing 10-20 pound
