The Intensive Outpatient Program is designed for clients who are stepping down from residential care or require an intensive level of care for those with a substance use disorder and/or mental health concerns. The Intensive Outpatient Program is approximately a 12-week substance use disorder treatment program that provides a minimum of 9 hours of services per week. Treatment is provided primarily through group therapy however, step-down services such as enhanced outpatient, relapse prevention, and aftercare recovery groups are also offered.
Duties Performed:
- Provide person-centered assessment, goal planning, and treatment for mental health and/or substance use issues.
- Provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines.
- Develop strength-based written treatment plans/updates and continuing care plans and make appropriate referrals.
- Complete written discharge summaries and correspondence within established timelines.
- Observe clients in a variety of situations.
- Assess diagnosis and mental status of clients, evaluate current level of functioning, and develop and implement treatment/service plan to address clients' needs.
- Intervene and de-escalate crises with clients.
- Provide crisis intervention via telephone or face-to-face in a variety of settings that may include crisis walk-in services, mobile on-site services, residential settings, etc.
- Assess the risk of harm to self or others and arrange needed resources.
- Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to provide services to clients.
- Maintain close and cooperative relationships with community agencies and other public agencies in carrying out treatment/service plans.
- Collaborate continuously with assigned clients to devise safety planning, provide symptom management, identify support systems, and reduce and mitigate risk factors.
- May deliver services to clients in their homes or other settings in the community.
- Assist individuals in improving their well-being and quality of life by training in behavioral skills;
educate clients to makeinformed choices;
develop clients' copingskills;
help clients to problemsolve and negotiate tasks of daily living.
- Learn, utilize, and support fidelity of any evidence-based practices within the assigned program to ensure effective client interventions.
- Perform case management services as needed to assist clients to meet their goals as stated in client service and/or treatment plans.
- Refer clients to internal and external supportive services such as vocational, medical, social services, and housing authority as well as residential, respite, and inpatient treatment.
- Maintain electronic health records following agency standards;
confirm coding of services rendered is ethically appropriate and results in successful billing. This may include entry into a partnering agency's electronic systems.
Education/Credentials:
- A Master's Degree in psychology, social work, or another behavioral health discipline from an accredited college or university is required.
- A Valid Driver's License and driving record that meets agency insurability requirements is required.
- Bilingual Skills (English/Spanish) are desired...
