Based in Hampden County, with your time split between the office and being out in the field, this is an engaging,
Rewarding job for a truly dedicated individual, interested in making a difference in the community and the lives of
Participants requiring permanent supportive housing. This is a 9 AM to 5 PM Monday through Friday position.
Position’s Essential Functions:
Service Planning:
- Development of a comprehensive assessment to better understand and engage participants regarding their
Unique strengths and experiences
- Utilizing a client-centered approach, assist participants in the development and implementation of an
Individualized action plan (IAP)
- In furtherance of their IAP, ongoing follow-up with participants to assist them in meeting their goals.
Housing Stability:
- Coaching and encouraging participants to maintain their units in a manner that is consistent with the lease and
Housing quality standards
- Assist participants in negotiation with the landlord for repairs and maintenance
- Coordinate with inspections to ensure units are inspected annually and on an as-needed basis
- Develop and maintain relationships with landlords
Financial Empowerment:
- Assist participants in achieving a sustainable source of income—employment, entitlements, etc.
- Educate and assist participants on how to access benefits and resources such as Social Security, EAEDC,
SNAP/Food Stamps, PVTA, and fuel assistance
Health and Wellbeing:
- Assist with scheduling primary care and psychiatric/counseling appointments to encourage support for healthrelated issues
- Facilitating adherence to medical and psychiatric/counseling appointments by connecting and teaching
Participants to leverage wrap-around services
- Provide transportation to participants to ensure they can address their medical/psychiatric and social service
Needs.
- Facilitating referrals to day programs and services to encourage meaningful community connections
Development/Additional functions:
- Attendance to required meetings, supervisions, and trainings
- Complete documentation requirements including Comprehensive assessments, progress notes
- Communicate and complete written documentation related to incidents promptly
- Perform other similar or related duties as assigned or as necessary.
Requirements
Required Knowledge, Skills, and Abilities
- A valid driver’s license, sufficient automobile insurance, an acceptable driving record, and a safe reliable vehicle
During working hours.
- Demonstrated ability to work independently, manage time, prioritize tasks, take initiative, follow through, and
Work cooperatively as a team member.
- Good oral, written, computer, and organizational skills.
- Demonstrated ability to adapt to the changing needs of the program participants.
- Strong interpersonal skills, patience, caring, and compassion.
- Must be at least eighteen years old.
Education and Experience
- High school diploma or equivalent (GED), bachelor's degree preferred
- Requires previous experience working with people with disabilities who have experienced homelessness, and/or
Substance use/mental health challenges
- Previous experience helping individuals obtain housing preferred.
- Bilingual preferred.
- Must be compassionate and empathic with a strong desire to help.
