Hospice Rn

Responsibilities:
Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
Admissions of patient for services and development of the initial patient/family Plan of Care
Coordination of services with the patient, family, and hospice interdisciplinary team members
Ongoing clinical supervision of Licensed Practical Nurses and Home Health Aids
Requirements
Valid Registered Nurse (RN) license
RN Diploma, Associates or B.S. degree in nursing
Minimum 1 year of Hospice agency experience required
Valid Driver’s License, auto insurance and reliable transportation
Excellent observation, problem solving and communication skills
Compassion, empathy and integrity
