Hospice Registered Nurse (Rn)

Touchstone Health of San Antonio is actively seeking a qualified and compassionate Hospice Registered Nurse (RN) Case Manager to care for our patients. The ideal candidate has excellent clinical skills, a passion for direct patient care, and the ability to function well autonomously. Able to commute in the San Antonio and surrounding area. Duties &
Responsibilities:
Evaluate and treat patients using the most current technology and practices
Develop initial plan of care based on communication with the physician and patient assessment
Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
Manage and educate Hospice Aides and LPNs performance in implementing nursing services
Collaborate with an interdisciplinary team to provide highest level of care
Requirements
Qualifications &
Requirements:
Current and unencumbered license to practice as a Registered Nurse in the state of Texas
Minimum of one (1) year of hospice nursing experience
Current CPR certification
Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
Exceptional customer care
Excellent written and verbal communication skills
MS Office skills (Word, Excel, PowerPoint)
