Full-time (40 hrs./week)
Salary:
$65,000.00 -$75,000.00 per year
Responsibilities:
Conduct patient admissions and assessments for home health services.
Develop comprehensive care plans based on patient needs and assessments.
Provide skilled nursing care, including medication administration and wound care.
Educate patients and families on home health care plans and self-care techniques.
Collaborate with the interdisciplinary team to ensure coordinated patient care.
Monitor patient progress, evaluate outcomes, and update care plans as necessary.
Document all patient care activities accurately and in a timely manner.
Maintain confidentiality and comply with all regulatory requirements.
Requirements:
Education:
Associate's or Bachelor's degree in Nursing (BSN preferred).
Licensure:
Active Registered Nurse (RN) license in the state of Pennsylvania.
Experience:
Prior experience in home health, community health, or a related field preferred.
Skills:
Strong clinical assessment and nursing skills.
