Job Description
OBJECTIVE:
Individual serves the public through effective implementation of federal/state regulations;
Service Coordinator provides Service Coordination on behalf of Family and Children First Council.
ESSENTIAL JOB FUNCTIONS:
Primary duties include a wide range of casework services to children and families. These duties may include any combination of any of the following:
Contacts referral source and/or family in a timely manner (e.G., within 5 business days) to explain the service coordination process and determine if the family would like to proceed with enrollment.
Service Coordinator orients themselves around the family’s experiences and helps other (i.E. services, supports, team members) to do the same.
During initial interactions with families, Service Coordinator actively listens to the family’s story and experiences and documents information that gives a better understanding of the family in astrengths-based needs driven manner.
Service Coordinator engages with youth and family to identify natural and professional supports to include in initial team meeting and plan development.
Service Coordinator facilitates discussion with each of the identified supports to explain the service coordination process, explain the value of their participation and invite them to the initial team meeting.
All activities are documented in OASCIS or an alternatively approved data system within 5 business days
Service Coordinator completes and reviews CANS (or other approved assessment) with the family and team to help determine and prioritize the strengths and needs of the family
Service Coordinator searches for and identifies strengths for the family during all interactions with the family.
Service Coordinator explores with family the services, supports, and interventions within the community, formal services, friends, and extended family that have made the most positive impact in their life.
During team meetings, Service Coordinator facilitates discussions with team members in how potential services, supports, and interventions can be utilized to create an individualized plan of care that focuses on the family’s needs and goals, builds off their strengths, creates shared action, responsibility, and ownership for all team members.
Service Coordinator works with family and team to collect data to help determine if services, supports, and interventions are making a difference for the youth and family.
Service Coordinator explores with family/team inclusion of potential community based and natural supports that can both help meet the family’s goals and build a sense of community (connectedness) around the family.
Service Coordinator meets with family/team members between team meetings to discuss if services and supports are being effective in helping the family meet their goals as well as identify any potential barriers that may exist.
Service Coordinator ensures that CANS (or other approved assessment) is updated every 90 days to help monitor overall family progress and aid in discussion with team around plan of care adjustments. Service Coordinator tracks overall progress of plan implementation, and update the plan as needed.
Service Coordinator shares results of collected data and progress monitoring efforts with family and team during team meetings and uses information to lead discussions concerning the overall impact of the established services, supports, and interventions and if they are assisting in meeting the family’s needs and goals.
Service Coordinator coordinates a transition plan with the family and team to help ensure the family is connected to future services and supports after disenrollment from Service Coordination.
Service Coordinator captures and summarizes positive changes in the youth and family’s ability to manage challenges and access necessary support.
Service Coordinator works to customize services and resources to the family and prepare to adjust thoseresources based on a family’s ongoing transition.
Service Coordinator guides the team in preparing a detailed strengths-based transition plan to support the youth and family when new challenges happen
Service Coordinator ensures family knows how to re- access Service Coordination in the future if needed
Other Duties:
Provide supportive services to the Children Services Division
The employee must possess the skills and ability to cope with diverse environments, health hazards and personal safety issues, including aggressive behaviors
