Enrollment Registered Nurse For The Pace Program

Our client is seeking an Enrollment Registered Nurse to join their team and play a crucial role in bringing the Program of All-Inclusive Care for the Elderly (PACE) to underserved communities in Los Angeles. As an Enrollment Registered Nurse, you will work closely with the Outreach Team to identify and assess prospective participants for the PACE program. Your responsibilities will include conducting home visits to evaluate eligibility, collaborating with the Interdisciplinary Team (IDT) and Outreach Specialist, and ensuring a smooth enrollment process for prospective participants.
Key Responsibilities:
- Collaborate with the Outreach Specialist and IDT to facilitate successful enrollment
- Conduct home visits to evaluate prospective participants for potential eligibility for nursing home level of care
- Perform home safety evaluations
- Participate in Enrollment Eligibility meetings
- Complete Nursing Facility Level of Care (NFLOC) assessments and maintain accurate documentation
- Support PACE Audits by DHCS and CMS
- Assist the Center Director in developing participant retention strategies
- Stay updated on eligibility, enrollment, and program specifications for the PACE Program
- Support business development and census growth by participating in outreach programs
Schedule and Shift Details:
- Monday to Friday, 8:
00 am to 5:
00 pm
- Mostly in-field home appointments with occasional onsite work at the Los Angeles clinic (up to 5 days per month)
- 95% travel to patient homes
