For the Community Supports position some duties include:
Maintain knowledge of individuals, their goals and support needs.
Identify skills and interests to help individuals be fully engaged with the community.
Develop an individualized plan of service with each person.
Document data and notes to adequately show individual’s progress toward goals on their service plan.
Attend and participate in scheduled staff meetings and complete required training.
Maintain positive relationships and good communication with co-workers, supervisor, and all other employees of Compass Career Solutions.
Transporting individuals to various community outings across county in personal vehicle.
For the Job coach position some job duties include:
Maintain knowledge of clients, their employment goals, jobsites, and support needs.
Assist clients to further their career growth and skills.
Assist clients in learning job tasks, adapting to the workplace environment, and establishing normalized work routines.
Participate in intakes, assessments, and Person-Centered Plans.
Document data and notes to adequately show individual’s progress toward goals on employment/ service plan.
Ability to use assigned technology as directed in the community.
Perform outreach to attract new clients and community partner
