We are seeking a Pharmacist with critical care or infectious disease training and background.To join our healthcare team.
The Advanced Clinical Pharmacist Critical Care practices the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values. They are responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in the ICU. They use advanced knowledge, skills, and professional judgment in all aspects of their work. They serve as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, they promote pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences contributing to publications, and providing regular, formal education to the pharmacy and interdisciplinary teams. Adherence to all regulatory and medication safety guidelines is at the forefront of all actions and decisions made within the practice of the Clinical Pharmacist
In this role, you will:
Participate in facility committees responsible for establishing and implementing medication-related policies and procedures within the ICU.
Primarily responsible for the direction of the critical care service line within the hospital.
Identify how drug costs may be minimized through the appropriate use of drugs in the ICU and through the implementation of cost-containment measures.
Participate in nursing, physician, or ancillary team onboarding and staff meetings with the ICU as appropriate to explain clinical pharmacy services and educate on new drugs or updated clinical guidelines
Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
Accurately compound and dispense pharmaceuticals.
Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
Sustain the hospital formulary, minimize nonformulary procurements, utilize therapeutic interchanges, and promote rational drug therapy selection.
Provide accurate and timely medication information to healthcare staff and patients.
Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as defined by Pharmacy Leadership.
Document all clinical activities and interventions accurately and completely.
Complete notification reports regarding medication errors and adverse drug reactions.
Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.
Qualifications
Education:
BS or Masters in Pharmacy
Graduate of a college of Pharmacy accredited by the American Council on Pharmacy Education
Graduate of an accredited PGY-1 residency program Required
Graduate of an accredited PGY-2 residency program Preferred
License/ Certifications:
Active pharmacist license in good standing with the appropriate State Board of Pharmacy
Experience:
3 years of acute care experience as a Pharmacist required
Five years of inpatient hospital experience Preferred
Three years of inpatient hospital experience Preferred
Minimum of 3 years experience in Infectious Disease/Critical Care in addition to a residency in Infectious disease or Critical care.
General Comments
Shift Time:
Mixed - Rotate every other weekend
Salary Range:
(1yr)-$56.55 (10 yr)- $ 63.13 (17yr MAX) $68.69
