Clinical Care Manager

Job Summary

The Clinical Care Manager plays a pivotal role in enhancing patient health outcomes through advanced nursing processes, care coordination, patient education, and team communication. This position is responsible for managing and optimizing the care of high-risk, acutely ill, and chronically ill patients, ensuring quality, continuity, and effective utilization of healthcare resources.

Key Responsibilities:

1. Care Management:

- Provide targeted, proactive care management to high-risk patients.

- Offer short-term care management for patients post-hospitalization or ED visits.

- Facilitate smooth transitions between care settings.

- Establish and maintain patient relationships with primary care providers.

2. Clinical Support:

- Deliver direct care management, including patient education and coaching.

- Collaborate with healthcare providers to set and achieve treatment goals.

- Monitor patient health status and adjust treatment plans as necessary.

3. Patient and Family Support:

- Perform comprehensive intake assessments.

- Address barriers to accessing healthcare and make appropriate referrals.

- Engage in patient and family education to promote self-management and lifestyle changes.

4. Data and Quality Management:

- Collect and report data on quality indicators.

- Participate in regular staff meetings and interdisciplinary care coordination.

- Prepare monthly updates on care management activities.

5. Departmental Collaboration:

- Work harmoniously with all departmental and organizational staff.

- Promote smooth delivery of services and participate in performance improvement activities.

- Utilize electronic software applications and manage related administrative tasks.

Essential Functions:

Patient Interaction:
Engage with patients and families to communicate care needs and barriers.

Resource Management:
Assist patients in obtaining necessary medications, equipment, and community resources.

Team Coordination:
Work with a multidisciplinary team to ensure coordinated and effective care.

Education and Training:
Identify training opportunities and share best practices with healthcare providers and staff.

Knowledge, Skills, and Abilities:

Expertise in population health and quality improvement tools.

Proficiency in MS Office Suite (Outlook, Word, Excel, PowerPoint).

Ability to maintain confidentiality and work with medically vulnerable populations.

Strong organizational, multitasking, and customer service skills.

Excellent written and verbal communication abilities.

Qualifications:

Experience:
3-5 years in healthcare, with at least one year in acute care nursing preferred.

Education:
Bachelor’s degree in Nursing (BSN/BAN) or related field (BS/BA).

License/Certification:
RN license in the State of Montana.
