Requirements:

At least 1 year experience in the past 3 years or at least 2 years experience in the past 5 years
Experience must be with comparable equipment (45 foot or longer)
No license suspension, revocation, or cancellation as a result of a moving violation or accident in the past 5 years
No major violations in the last 3 years
No more than 2 jobs in the last year;
no more than 4 jobs in the last 3 years
No failed or refused DOT Drug or Alcohol test ever
