Position Summary
Supervises and coordinates resident nursing care on the Subacute Unit.


Primary Duties
1. Knowledge of HDMHD policy/procedures, nursing certification requirements for own and others positions. Performs own job duties and ensures that the supervised staff perform their duties within legal and professional regulations and guidelines. Ensures that self and those supervised apply nursing principles, techniques and practices according to HDMHD policies and procedures. Promotes professional environment of care.
2. Demonstrates a understanding of Title XXII, OBRA, JCAHO and MediCal Subacute Program requirements applicable to SNF and MediCal certified Subacute ventilator programs. Performs all skills appropriately with consideration to age specific needs of young adult, middle-aged adult, and older adult residents.. Residents are treated with sensitivity and appropriateness in approach to age, culture, religion, gender, and sexual orientation of the residents.

  • Maintains current professional license (RN) and appropriate CPR certification and provides facility with a copy of these documents in a timely manner to ensure practice is covered by active license and certification at all times. Ensures keeps up-to-date educationally especially in areas of specialty.
  • Completes initial and annual competencies and annual health and safety requirements timely. Assist Clinical Director as assigned to assess initial and annual skills competency for LVN's and CNA's. Orients new RNs as assigned. Assists Clinical Director in ensuring that LVN and CNA staff receives appropriate orientation. Ensures that Registry and float staff receive appropriate orientation before starting work, and that staff are supportive of Registry and float staff
  • Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations. Observes Residents Bill of Rights and reports incidents of elder abuse, if indicated. Ensures supervised staffs also are aware of and follow these regulations. Ensures self and staff carry out their duties in a manner sensitive and appropriate to age, (primarily adults and older adults) culture, religion, gender, and sexual orientation of the residents.
  • Controls the environment to prevent hazards. and cross infection by observing HDMHD policies and procedures. Ensures supervised staff follows HDMHD policies and procedures pett-aining to preventing hazards and cross infection. Ensures alternatives to physical and chemical restraints are attempted before use of physical and chemical restraints. Is familiar-with MSDS information sources-and acts as a resource for supervised staff
  • Communicates with Physicians, our own and other medical facilities, other departments within the district, directly supervised staff, residents, family members and visitors, to ensure a professional two-way flow of information regardingresident care and other issues. Assesses residents that supervised staff or family members have concern about, and make appropriate-decisions that demonstrate sound nursing judgment and decision making skills. Communicates regularly with the Clinical Director regarding resident:
    Care and other issues. If needed, gives verbal counsel.Ing to supervised-staff, and-notifies the Clinical Director of thisand other disciplinary issues.
  • Assists Clinical Director in ensuring department operates in a financially sound manner by supervising staff use of supplies and resources_ Appliesssameprinciples to assist in productivity by.Staffing appropriately.

Minimum Education/Certification:
· Current California Registered Nurse License (RN)
· BLS Certification
· 6 months previous experience in a Sub acute Ventilator Unit, Acute Hospital experience or an acquired equivalent competency in caring for people in a Sub acute Ventilator Unit setting.
· Previous experience in a Charge or Supervisory position preferred

Pay range:
$49.69 - $77.02 hourly

**Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
