Case Manager | Youth

The April Parker Foundation, a 501(c)3 community-based non-profit organization, is seeking a Program Coordinator to join our team. As a Program Coordinator, you will play a crucial role in helping uplift underserved communities through culturally relevant and highly supportive services. Responsibilities
The APWorks Futures First Case Manager (youth) will identify, screen, and assist Youths and Entrepreneurs with educational initiatives aimed at providing social-emotional support and workforce pathways for persons within the City of Long Beach.
This person will work in collaboration with the APWorks Program Manager and the APWorks Program Coordinator to conduct community-linked youth outreach and execute marketing plan, outlining specific engagement of high-need populations, and residents of priority communities (e.G., Cambodia Town, Coolidge, Ramona Park, Washington, and West Long Beach).
One of the core roles of the APWorks Futures First Case Manager (youth) is to create new training presentations and implement the right development tools with a core focus on Special Education Instructional Aid and Behavioral Aid for students and adults with Developmental disabilities while improving the existing materials from the NFTE program.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:

Training Material:
Work alongside the APWorks Program Manager to create “Best in Class” Development Tools.
Outreach:
Implement community-linked youth outreach and marketing plan, outlining specific engagement of high-need populations, and residents of priority communities.
Eligibility Screening:
Confirm out-of-school and out-of-work status.
Communication with Retreat Team:
Maintain effective communication with the retreat team.
Wellness Activities:
Host weekly or biweekly wellness activities, workshops, or events to foster a supportive and engaging environment for the opportunity youth.
Co-Case Management Meetings:
Attend a 30–60-minute co-case management meeting on a weekly basis to discuss progress, challenges, and strategies for enhancing program effectiveness with the care team which includes Economic Development Department and Health Department
Program Meetings:
Attend a weekly one-hour program meeting with the two City departments to collectively discuss ongoing program deliverables, timelines, learnings, and overall partnership of the three entities.
Information Sessions:
Organize information sessions to actively reach out to potential opportunity youth and provide them with details about the program's benefits and offerings.
Tracking &
Evaluations:
Work with the program’s third-party evaluator to track data on program’s Opportunity Youth.
Minimum Qualifications (Knowledge, Skills, and Abilities)
Masters, Secondary Education;
Literacy and Language Arts (preferred)
Bachelor of Arts, Communication Studies;
Minor:
Sociology
A minimum of two years of related experience OR
Ability to understand the characteristics of a wide range of intellectual disabilities.
Personal and professional experience with mindfulness or meditation work is preferred.
Possesses excellent interpersonal skills and the ability to handle emotionally charged situations.
Ability to effectively communicate with external community partners, and clients verbally and in written correspondence.
Technical proficiency with Unite Us platform.

Develop and implement culturally relevant programs and services to meet the needs of underserved communities
Coordinate program outreach efforts to increase community participation and engagement
Collaborate with community partners to leverage resources and build strong relationships
Conduct regular needs assessments to identify gaps in services and potential program enhancements
Monitor program outcomes and evaluate effectiveness to ensure continuous improvement
Manage program budgets and ensure financial stewardship according to the foundation's guidelines
Supervise and support program staff, including recruitment, training, and performance evaluations
Requirements
Bachelor's degree in a related field (social work, non-profit management, etc.)
Previous experience in program coordination or related roles within the non-profit sector.
Strong organizational skills with the ability to multitask and prioritize competing demands.
Excellent communication and interpersonal skills, with the ability to build relationships with diverse stakeholders.
Knowledge of community resources and programs that support underserved populations.
Proficiency in Microsoft Office Suite and other relevant software applications.
Demonstrated commitment to the organization's mission and values.
Flexibility to work evenings and weekends as needed for program activities and events.
