Responsible for the provision of case management services to the alumni of Cal Farley’s Boys Ranch to include coaching, goal setting and tracking, maintenance of files and related activities. Serves as liaison between clients and outside social service resources and agencies.
Requirements
Essential Duties:

1. Exhibits regular and punctual attendance.
2. Completes all assessment tools indicated by Alumni Support Services protocol to promote valid and reliable outcome measures.
3. Actively participates in discharge planning and development of aftercare plans for residents of Boys Ranch.
4. Assists with case management and program development of Alumni Support Center.
5. Maintains case files that are current, accurate, confidential and in compliance with all Council On Accreditation (COA) standards.
6. Prepares of Plans of Service for clients who are receiving alumni support services.
7. Provides at least two monthly in-home visits with each client receiving services, documents this contact and communicates the results with other staff members as appropriate. Contacts all clients on assigned caseload a minimum of three times monthly.
8. Provides follow-up and aftercare services as assigned.
9. Provides life skills, coping skills, social skills and related education groups as assigned.
10. Provides transportation to clients for referral appointments with various community agencies (medical, dental, psychological, etc.)
11. Provides crisis intervention for clients to include the use of approved verbal and physical intervention techniques.
12. Establishes and maintains communication with other community-based services administrative staff to coordinate case management services.
13. Assists in the implementation of the Alumni Support Center program in accordance with Cal Farley’s Model of Leadership and Service.
14. Exhibits appropriate interactions with all clients.
Job Specifications:
1. A Bachelor’s degree from an accredited university in a human services field is required.
2. Related professional license is preferred.
3. A minimum of 12 months family case management or casework experience is required.
4. Must successfully complete competency checklist.
5. Basic competency in Microsoft Office for Windows applications (word processing, database, spreadsheet, e-mail, presentation graphics, etc.) is required.
6. Bending, lifting, grasping, fine hand/eye coordination, pushing/pulling, prolonged sitting/standing.
7. Proficiency in general office equipment operation is required.
8. Must possess exceptional time management and detail-orientation skills.
9. Must be at least 21 years of age.
10. Must possess a valid Texas driver’s license.
11. Must be available for travel as needed.
