Job Description
The Behavioral Health Case Manager serves as a member of a multi-disciplinary patient care team for NATIVE HEALTH’S Behavioral Health Department. The Behavioral Health Case Manager provides case management services that support the treatment goals of the patients care plan. The Behavioral Health Case Manager represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH, always exercising utmost discretion, diplomacy and tact in customer transactions.
KEY ACCOUNTABILITIES:
Provides non-intensive case management services in support of Behavioral Health patients and providers.
Supports the behavioral health program and its team members.
Participates in quality improvement and quality assurance activities for the department.
Supports the organization as needed
