The Advanced Practice Provider (APP) provides care in the acute care inpatient setting based on educational preparation and certification in either the APN or PA role specific to the transplant patient population. Key responsibilities include coordinating patient care within the Department of Abdominal Transplant Surgery, collaborating with attending physicians, resident house staff, Advanced Practice Providers, and other multidisciplinary team members. As an expert practitioner, the APP performs selected invasive procedures, designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APP facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching.

APP essential job functions:

1. Functions independently and collaboratively with the physician to provide continuity of health care services for individuals and support to their, families.

2. Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment. Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, identifies variations from the norm in diagnoses emergent problems and complications.

3. Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution.

4. Facilitates care coordination through multidisciplinary rounds and implements goals of care from pre-op through hospital discharge.

5. Remains compliant with regulatory bodies and participates in quality improvement processes for the transplant programs.

6. Provides education to patients and families related to transplant care and related healthcare decisions.

Shift:
Rotating day (6a-6p) and night (6p-6a)

  • As part of the abdominal transplant service, the APP will provide coverage for the abdominal transplant patients which covers a 365 day/24 hours schedule. The expectation in this role is that day/evening/holiday shifts will rotate among a core group of 7.0 FTE APPs.
  • Coverage expectations:
    Holiday and evening shift coverage will rotate by each APP on an annual basis. Evening shifts (6pm to 6am) will be divided with the expectation of covering 4-6 per month for each full-time APP depending on the staffing and patient needs.

Minimum Education:
Required:
Masters Degree as either Acute Care Nurse Practitioner or Physician Assistant

Minimum Experience:

Required:
At least 1 year of inpatient APP experience preferred

Preferred:
2-5 years of previous job-related experience preferred

Licensure/Certifications:
Required:

CPR (Cardiopulmonary Resuscitation)

ACLS

Current Registered Nurse License State of Illinois, Nurse Practitioner License State of Illinois

Must have or be able to qualify for Illinois and DEA prescribing numbers
